     I Hope you enjoyed your summer. It's been a busy one for us at VRF, So there's lots to report In this month's newsletter. First, Let's talk about some news on early treatment diagnosis, which is of critical importance as early treatment produces better and long lasting results. Recent research has shown that a hand-held 'black light'. which is commonly used to locate stains around the house or find counterfeit currency , can also detect early-stage vitiligo. This is instead of a specialized wood's lamp, which is bulky and expensive. 

     Talking of research, Recently, we did some of our research into "traditional medicines' and other possible ethono-pharmacological shortcuts in the research for effective treatments. Indigenous cultures possess notions, to a remarkable extents, of how different plants can be used to treat skin diseases. Of course, plant treatments need to be approached with caution, given issues of possible toxicity, so we have published an FAQ section on this subject on our website.

     Now for some support news.Larry (A.K.A Spotty) King Has recently launched vitiligomatch to help the community. This social site is a place where people with vitiligo and their friends and supporters can chat, exchange photos and generally 'fell at home'. Make sure you take a look.

MELBILD

Melbild          

 For The Use Of A Registered Medical Practitioner Or Hospital Or Vitiliogo Patient. 

 

Melbild is a tropical preparation for management  of vitiligo. It is Available in two Strengths and use for external use only.

 1.  What is Melbild ?
 =  It is a sequence of amino acids derived from basic fibroblast growth factor (bFGF). It Stimulates growth  of melanocytes and therefore helps in repigmenting the depigmented skin.

With the use of such preparations one can expect some noticeable change in about 3-6 months.
Significant repigmentation may be achieved with continued use for about a year.


 2. How Is MELBILD To Be Used ?
 = To Begin with , Remove the plug of the MELBILD container and replace it with a roller. Gently, Shake the bottle , and then apply. Before applying MELBILD, Clean and dry the affected area thoroughly, and then apply as a very thin film.
It is best to apply MELBILD some 2 hours before retiring for the night. Every morning, the treated patches may be exposed to sunlight for 10-15 minutes. If this not possible, the treated patches may be exposed to commercially available Ultraviolet Lamps.

3. How Is MELBILD To Be Stored ?
= MELBILD is to be stored at a temperature below 25 degree Celsius and away from light, to maintain optimal effectiveness.

New Medicine for vitiligo (CXCL 10) . The Light of hope.

New Medicine for vitiligo. MUST READ :

In the lab , Experts focus on two aspects of Vitiligo :-

Firstly , They study how abnormal , "stressed" melanocytes alert the immune system to their presence. Once stressed , melanocytes produce signals , that recruit T cells to the skin , which then find the melanocytes and kill them.

Secondly , They study how the T cells detect these signals , enter the skin , find the melanocytes , and kill them.

For the seek of vitiligo patients , experts use mouse as model , that had white spot on there body as shown in figure :-
Examining these mice from many angles , their disease looks very similar to human vitiligo . The benefit of this system is the powerful tools available to study diseases in mice. They are developing humanized mouse model of vitiligo ,  where they transfer human T cells , or both T cells and skin , from vitiligo patients to immunosuppressed mice that permit the growth of human tissues. The benefit of this system is the ability to study human T cells and skin and it has the potential to test new treatment on human tissues before attempting clinical trials.

 They are conducting a clinical trial to investigate new drug as treatment for vitiligo :- 
They were able to identify one of the critical pathways used by T cells to crawl into the skin and find melanocytes. The first signal in the pathway is IFN-g , a protein made by immune cells , which act as a powerful master switch to turn on immune responses. IFN-g then turns on CXCL10, which is required for the ability of T-cells to find melanocytes and kill them. Imagine , an ant that has found a new piece of food on the ground. The ant then lays a chemical trail that others follow to the food.CXCL10 acts acts like that chemical trail , promoting both proper   localization of T cells to Find the melaocytes , and also increase their determination to kill them. There are existing drugs that have been developed by pharmaceutical companies that block the ability of CXCL10 to function in humans, and so they may be effective treatments in vitiligo as well.
They have also begun to identify the signals produced by stressed melaocytes that activate immune cells. We believe that studying the communication between stressed melanocytes and immune cells will tells us how vitiligo gets started in the first place, and how to better prevent its onset and spread.

VITILIGO

About Vitiligo :- Symptoms , Effects ,Transmission . 

Skin gets its color from melanin in its uppermost layer , or epidermis. That melanin is produced by cells called melanocytes are destroyed or fail to function properly , the body cannot produce melanin , and the skin loses its color. This anomaly does not happen overnight , nor does it happen evenly throughout the body . Vitiligo can occur in three different forms :-

Focal , in which loss of color is limited.

Segmental , in which one side of the body loses color

Generalized , in which the symptoms are widespread.

Because lightening of the skin  is  more noticeable in people with darker complexions , Mcheal Jackson's case obvious both to him and to the public . But vitiligo can affect people of any skin tone. Between 0.5 to 1 percent of  the world's population has the condition , which affects people of all races and both sexes equally  .Symptoms are more likely to begin when a person is in his 20s . The most common starting points for the telltale white patches are above the eyes or on the neck , armpits , elbows , genitalia , hands or knees. Less common signs of vitiligo are premature graying of scalp hair and loss of color in the mucous membranes or retinas .

While doctors donot know why some people get vitiligo , they theorize that it is an autoimmune disease , a disorder in which the body attacks its own healthy tissue for unknown reasons . Heredity plays a role , although many people with vitiligo donot have a family member with the condition . Doctors believe a combination of genetic , immunalogic , neurogenic and environmental factors are at play in many cases . Some people first report seeing the white patches after severe sunburn (one more reason to always protect your skin by wearing sunscreen) , while others believe their vitiligo was triggered by emotional trauma .  But is this condition contagious ? No. This disease isnot contagious.

World Vitiligo Day 25th June

World Vitiligo Day - 2013(WVD)

World Vitiligo Day 2013 was made extra special this year by the participation of support groups and organizations from all over the world , but it was made extra special by the participation of individuals who , in many different countries , really went all out do their bit to help spread awareness and raise the profile of the campaign.Obviously , as a non profit advocacy organization , we had very little funding to work with and couldn't make T-shirts , but it didn't matter as the WVD T-shirt challenge got a fantastic response from literally hundreds of people all over the world , sporting their own WVD T-Shirts to help spread the message. Thank you to everyone that printed , designed , made their T-shits , you are all awesome.

Last year World Vitiligo Day was focused on rome and had a great response and this year in Detroit the Henry Ford Hospital and vitiligo treatment centre team with Dr.Henry Lim and other many more Vitiligo support groups and vitiligo Research foundation really managed to put together a wonderful event that really did make it a special and high profile day.

The press conferences was held , In  recognition of the efforts of those in Detroit to raise vitiligo awareness , the mayor of Detroit and the governor of Mishigan were both moved to give official proclamations declaring the Detroit and the state of  Michigan Officially recognize World Vitiligo Day.

Support group from all over the world held World Vitiligo Day support meeting and events including events in puerto Rico , USA , South Africa , UK , France and Germany .In Boston , Following World Vitilgo Day the Boston city Council delivered a resolution in support of the VITFriends efforts in increasing awareness and offering education about Vitiligo.

Through out this year there will be events focused on awareness and also focused on research and filling new treatments happening all over the world . We will be keeping track of these  as they happen and keeping everyone up to date.   World Vitiligo Day 2014 will be coming from Sao Paolo in Brazil .

I use melgain(liquid) every night 1-2 hours before going to sleep and in morning 15-20 minutes sunbathing is important . Mesoral-10 is tablet that i take 3 days a week and after having tablet , UVB is necessary after 2 hour . Medrol(16 mg) is also a tablet i take 2 days a week. Every morning after sunbath Metasone cream is layered over white patches. This is my routine and i have adopted to  these medicine. Consult with your doctor about these medicine.

UV Ray Phototheraphy  :-

1.Narrowband UVB :- Now considered the gold standard of treatment for vitiligo covering more than 20% of the body . Narrowband UVB (NB) uses the portion of the UVB spectrum from 311-312 nm. This region has been determined to help stimulate pigment cells to produce melanocytes in less time than it takes to burn the skin . Any kind of light therapy has a suppressive effect on the immune system , so it can possibly stop new areas from forming as well. NB can be done in the doctor's office with a full-body cabinet or, with a doctor's prescription , from home using a full-length panel or a handheld device . The handheld are very convenient for small areas of vitiligo but are too tedious and cumbersome to use for larger percentages as they cover a very  small area at a time . NB is sometimes used in combination with other topical treatments, but is effective for many on its own. NB can be used on children old enough to stand still and keep goggles on . Results are often seen beginning between 30 to 60 treatment is usually given 3 time a week. Potential side effects of NB include burning if used for too long. If using a full-body panel or box , the normal skin may tan , increasing the contrast . Only full sized body units provide the immune suppression needed for stabilization that may halt further pigment loss , a the handheld units only treat isolated areas . As long as the patient is old enough to understand that they must keep their eyes closed at all times , and has as annual eye exam , research has shown that it is safe to expose the eyelid to NB-UVB light . The exception to this would be if a psoralen had been used.

2. Psoralen Plus UVA Light (PUVA) :-

 Formerly the gold standard of treatment , PUVA has mostly been surpassed by NB-UVB as NB-UVB is at least as effective and has fewer side effects .  The psoralens are typically taken orally ,  but can also be used topically . A UVA light box or sun may be used to provide the light  component . Melaginina that is available in countries outside the US is a psoralen and must be used with caution and under a doctor's care .Treatments are usually given 3 times per week . Side effects of PUVA include burning , stomach upset , liver issues , and cataracts .  To prevent the possibility of cataract formation you must wear sunglasses , which protest your eyes against UVA , for 12-24 hours after taking the tablets. Sunglasses labeled uv 400/UVB/UVA protection 100% are recommended. PUVA  is used less often in children because of the risk of side effets.

Mometasone Furoate Cream USP 0.1:-

Mometasone Furoate Cream USP , 0.1% contains mometasone furoate , USP fordermatologic use. Mometasone furoate is a synthetic corticosteroid with anti-inflammatory activity.

Chemically , mometasone furoate is 9alpha ray,21-Dichloro-11beta ray , 17-dihydroxy-16alpha ray-methylpregna-1,4-diene-3,20-dione 17-(2-furoate), with the empirical formula C27H30cl2o6 , a molecular weight of 521.4 and the following structural formula :

Mometasome furoate is a white to off-white powder practically insoluble in water , slightly soluble in octanol , and moderately soluble in ethyl alcohal.Each gram of mometasone furoate cream USP , 0.1% contains 1 mg mometasone furoate , USP in a cream base of hexylene glycol , phosphoric acid , propylene glycol strearate , stearyl alcohol alcohol and ceteareth 20 , titanium dioxide , aluminum starch octenylsuccinate , beeswax ,white petralatum , and purified water .

CLINICAL PHARMACOLOGY :-

Like other topical corticosteroids , mometasone furoate has anti-inflammatory , anti-pruritic , and vasoconstrictive proporties. The mechanism of the anti-inflammatory activity of the topical steroids , in general , is unclear . However , corticosteroids are thought to act by the induction of phospholipase A2 inhibitory proteins , collectively called lipocortins . It is postulated that these proteins control the biosynthesis of potent mediators of inflammation such as prostaglandins and leukotrienes by inhibiting the release of their common precursor arachidonic acid. Arachidonic acid is released from membrance phospholipids by phospholipase A2.

Dosage and Administration :-

Apply a thin film of mometasome furoate cream USP , 0.1 % to the affected skin areas once daily . Mometasome furoate cream USP , 0.1 % may be used in pediatric patients 2 years of age or older . Since safety and efficacy of mometasone furoate cream USP , 0.1%   have not been adequately established in pediatric patients below 2 years of age , its use in this age group is not recommended . As with other corticosteroids , therapy should be discontinued when control is achieved . If no improvement is seen within 2 week , reassessment of diagnosis may be necessary . Safety and efficacy of mometasone furoate cream USP , 0.1% in pediatric patients for more than 3 week of use have not been established.

Methylprednisolone is a steroid that prevents the release of substance in the body that cause inflammation. Methylprednisolone is used to treat many different inflammatory conditions such as arthritis , lupus , psoriasis , ulcerative colitis, allergic disorders , gland (endocrine) disorders , and conditions that affect the skin , eyes , lungs , stomach , nervous , system , or blood cells.

Methylpredisolone may also be used for purposes not listed in this medication guide. You should not use methylprednisolone if you are allegic to it , or if you have fungal infection anywhere in your body.Methylprednisolone can weaken your immune system , making it easier for you to get an infection. Steroids can also worsen an infection you already have , or reactivate an infection you recently had. Tell your doctor about any illness or infection you have had within the past several week.

To make sure methylprednisolone is safe for you , tell your doctor if you have ;

Heart disease , High blood pressure , Kidney or liver disease (especially cirrhosis); a history of tuberculosis ; a parasite infection (pinworms); Glaucoma or cataracts ; a recent head injury; Stomach ulcers , ulcerative colitis , or diverticulitis ; a thyroid disorder; low bone mineral density ( steroid medication can increase your risk of bone loss) ; depression or mental illness ; a muscle disorder such as myasthenia gracis ; seizures or epilepsy.It is not known whether medrol will harm an unborn baby. Tell your doctor if you are pregnant  or plan to become pregnant while using this medication.

Merhoxsalen (also called xanthotxin , marketed under the trade names Oxsoralen , Deltasoralen , Meladinine ) is a drug to treat Psoriasis , eczema , vitiligo , and some cutaneous lymphomas in conjunction with exposing the skin to UVA light from lamps or sunlight . Methoxsalen modifies the way skin cells receive the UVA radiation , allegedly clearing up the disease. The dosage comes in 10 mg tablets ,which are taken in the amount of 30 mg 75 minutes before a PUVA (psoralen + UVA) light treatment . Chemically , methoxsalen belongs to a class of organic natural molecules known as furanacoumarins. They consist contents :-

# Natural occurrences :-
Methoxsalen is extracted from Ammi majus , a Plant of the family Apiaceae. The substance is also present in other Apiaceae Biosythesis.
The biosynthetic pathway is a combination of the shikimate pathway , which produces umbelliferone , and the mevalonate pathway.

Umbelliferone is a phenylpropanoid and as such is synthesized from L-pheylalanine , which in turn is produced via the shikimate pathway. Phenylalanine is lysated into cinnamic acid , followed by hydroxylation by cinnamate 4-hydroxylase to yield 4-coumaric acid. The 4-coumaric acid is again hydroxylated by cinnamate/coumarate 2-hydroxy to yield 2,4- dihydroxy-cinnamic acid (umellic acid) followed by a bond rotation of the unsaturated bond adjacent to the cardoxylic acid group. Finally an intermolecular attack from the hydroxyl group of c2 to the carboxylic acid group closes the ring and forms the lactone umbelliferone.

Dimethyallyl pyrophosphate.
The biosynthetic route then continues with the activation of dimethylallyl pyrophosphate (DMAPP) , produced via the mevalonate pathway , to form a carbo-cation via the cleavage of the diphosphates . Once activated , the enzyme umbelliferone 6-prenyltranferase catalyzes a C-alkylation between DMAPP and umbelliferone at the activated position ortho to the phenol , yielding demethylsuberosin. This is then followed by a hydroxylation catalyzed by the enzyme marmesin synthase to yield marmesin. Another hydroxyation is catalyzed by psoralen synthase to yield psoralen .A third hydroxylation by the enzyme psoralen 8-monooxygenase yields xanthotoxol which is followed by a methylation via the enzyme xanthotoxol 0-methyltransferase and s-adenosyl methionine to yield methoxsalen.

# Risks and side effects :-
Patients with high blood pressure or a histroy of liver problems are at risk for inflammation and irreparable damage to both liver and skin . The eyes must be protected from UVA radiation . Side effects include nausea , headaches , dizziness , in rare cases insomnia. Methoxsalen has also been classified as an IARC Group 1 carcinogen but only cancerous when combined with Ultra Voilet-A radiation.

Vitiligo/Leucoderma/white patches of skin are an acquired loss of pigment resulting in white patches of skin . It affects all races and is referred to as an acquired idiopathic , macular , total loss of pigmentation of skin . This disorder is wide spread and its incidence may be higher in india , Asian and middle east countries.


Melgain is uses for vitiligo (for external use only). Local application of melgain on vitiligo patches produces normal pigmentation by stimulating the multiplication of pigment producing cells ,melanocytes , at the root of hair and at the edges of the vitiligo/white patch and their migration. Melgain is effective alone or in combination with other modules of treatment . No contraindications or side effects have been reported. The local application of melgain on punch grafted vitiligo patches also accelerates re-pigmentation many fold.

Melgain is made up of :- Each ml contains Deca-peptide-1mg (Isopropyl alcohol , Myristate and Glycol-Q.S.)

How to use =>

1. Melgain is presented in a glass bottle with Gufic roller (Separately packed in a pouch).

2. Gently open out the 'teflon plug' the comes along with the bottle.

3. Hold the bottle firmly and replace the plug with the roll on provided in the pouch(placed inside the cartoon).

4. After ensuring proper fixing of the roll-on,gently move the roll on to apply the lotion on the affected area.

Result  =>

With the above regimen the pigmentation response can be noticed in most cases in about 3 to 6 months in the form of pigmentation at the boarders or inside the patches as dots of pigment. Significant re-pigmentation in  about  80 % of patients has been reported after one year of treatment as mono-therapy.For more fast result exposure to sunlight or the treated patches may be exposed to UV lamps which are commercially available.

  VITILIGO WITHIN ME

 

Vitiligo  is disease that effect skin then force pigmentation of skin to fail and white patches are created . There is no care....... that's not true . I am vitiligo patient and my hobby was to be soldier but due to vitiligo my aim just cracked.Here goes my story and how i am able to fight back?

I don't want to break my aim and myself . I was about 13 when i got white patches on my back and neck. Doctor told me it was 'Fungal Infection' so they suggests me"Ketonal"(fig.1). It was great Shampoo(Ketonal) within 3 weeks all thing was back with me but the action was not over, when i was 14 ,  again white patch knock my body this time it break inside me from my leg. I tried same medicines as before but this time it failed totally. My sister used to work at Skin hospital so, she take me for check up. First of all, Doctor said"I was in initial stage so, not to worry." . He provided me with herbs medicines named 'pigmento' and 'Ludoil' both have bad smell , i was 14 so white patches was no problem for me. I was unaware that white patch was increasing . Suddenly, i get cut in leg then it didn't heal instead it leave huge white patch on my leg(fig .b). Still, non of family including me was that such serious.

Few year past away. While i was bathing i notice that i got white patches and i started counting them. There were 13(2008) white patches(fig .c). Now i was full of fear so, i ask my mom (soft voice) "I got white patches on my leg and chest." Mom makes fun of mine, "Not to worry boy , Girl will like you, Anyway girl like white boys." I just shy and ran to upstairs . My elder sister was  busy in laptop so, i said to my another sister about white patches she wish to have a look then she notice that it was increasing faster and faster so, she decided to take me to Doctor again. This time Doctor was more serious i was able to notice that from his body language.

How the regimentation occurs

Doctor said "These are vitiligo (dhubi)."   I wasn't scared nor anybody in that room but i just came to know new guest name of my body.Then i started to have high medical care that include 3 tablet (all sucks) , 2 creams and U.V ray.I am still using these medicine and it's been about 2 months(2013) and Now i am able to  see brilliant result but uh i still think my aim to be soldier will crash anyway , better i grab good aim . What you say guys?
